Puppy Sales Contract
This sales contract entered into on this _____ day of_________, in the year of _______, between Laura Balis (hereinafter referred to as “Seller/Breeder”) and ______________________ (hereinafter referred to as “Buyer”). For the sum of $_______.

This puppy’s health is guaranteed to be good to the best of our knowledge at the time of sale and has had a routine health examination by a licensed veterinarian. Buyer must have the puppy examined by a licensed veterinarian within 72 hours of delivery, or within 3 business days. If buyer does not have the puppy examined by a licensed veterinarian in the allowed time contract is considered null and void._____Buyer Initials.
Said puppy is sold with;
___Limited AKC Registration and a Spay/Neuter contract. No AKC paperwork will be given until written proof is received that said puppy has been spayed/neutered by a licensed veterinarian by 6 months of age. If said puppy is not spayed/neutered in the given time, this contract will be considered null and void._____Buyer Initials.
___Full AKC registration with breeding rights. Buyer agrees that any future offspring of said puppy will not be sold to pet stores, dog farms, puppy mills, wholesalers, puppy brokers or be taken to the pound._____Buyer Initials.
It is agreed and understood that this puppy has not been purchased for resale or for use by any wholesale operation. The Seller/Breeder has the right to have the first choice of resale and should be informed before any other contacts are made. Buyer agrees that if for any reason, the new puppy does not work out, the puppy is to be returned to Seller/Breeder only or suitable pet home and never to a pet store, dog farm, puppy mill, wholesaler, agent, or the pound._____Buyer Initials.
Buyer agrees that puppy is to be housed indoors as a member of the family, and is not to be kept on a chain, or in a crate for extended periods of time._____Buyer Initials.
ScallyWag Shih Tzu’s are guaranteed against genetic (inherited) life-threatening medical conditions that is properly diagnosed by a licensed veterinarian and substantiated with proper testing, within the first 1 year of life. Seller/Breeder should be notified immediately via telephone and in writing within 48 hours of diagnosis. Seller/Breeder reserves the right to request a second opinion by another veterinarian at the Buyer’s expense. Buyer must keep puppy current on vaccinations, and routine medical care & feed their puppy a quality dog food (Eukanuba or other premium non-grocery store brand) or Seller/Breeder will not honor these guarantees.

In the event a life-threatening medical condition proven to be genetic occurs within first 1 year of puppy’s life, Seller/Breeder will replace the puppy with one of equal or greater value at earliest availability. Selection of replacement will be made by Seller/Breeder. Seller/Breeder reserves the right to make the decision as to whether the puppy is returned to Seller/Breeder, or allowed to remain with Buyer._____Buyer Initials.
In the event any part of this contract is not adhered to, the guarantee is null and void. If the puppy is bred within the first year the guarantee is null and void. Any legal action instigated by Buyer relating to the treatment of this animal will be the sole financial responsibility of Buyer and shall be filed in Salt Lake County, Ut. _____Buyer Initials.
Any further conditions for this sale shall be as follows. Seller/Breeder guarantees veterinarian check up, current vaccines, & microchip._____Seller Initials.
No refunds of deposits will be given, nor will the purchase price of the puppy be refunded for any reason once the puppy is in the possesion of the buyer.
An additional $275.00 is required if puppy has to be shipped.
I agree with the above contract and will hereby abide by the stipulations mentioned.
